I know that NYC required improved ventilation as part of its building codes in the early 20th century to fight tuberculosis. That's why public schools, for example, had those high ceilings. It's also why so many NYC apartments are overheated. The code required that the room be suitably warm even when the windows were open in the winter. That meant installing enough steam heating capacity for this to work. When people close their windows, they get overheated.
